Project Request Form

Our NEW Project Request Form is for ANYONE to fill out that has an idea for a project in their community. This very simple, two-step form was created to help get all the necessary information for a project, first hand. The form will ask about the organization, information regarding the proposed project, goals for the project’s future, and matching funds. The entire form MUST be filled out. Once the form is completed, email it back to SICOG. We will then review the form, and designate the appropriate planner to connect with you. If you have any question, please call our office.
